Abstract

Title: The Effectiveness of a Cardiac Rehabilitation Training Program Based on Continuous Care model on Empowerment and Cardiovascular Indices in Patients with Ischemic Heart Disease Introduction: Considering the importance of cardiovascular rehabilitation programs and lack of sufficient information on the effects of cardiac rehabilitation in patients with ischemic heart disease in Iran, research in this field is important and its findings can be evaluated in the evaluation of cardiac rehabilitation programs in Iran.. The results of this study are useful in highlighting the importance of cardiopulmonary rehabilitation training in order to optimize the physical, psychological and social performance of cardiac patients following ischemic heart disease. Methods: The present study is a clinical trial which is a research population of patients with ischemic heart disease who have been admitted to the heart of the hospital in Sabzevar hospital. A sample of 70 patients with ischemic heart disease admitted in cardiac and CCU were easily accessible and randomly assigned to two groups of control and intervention. The data gathering tool was a three-part questionnaire. The first part was demographic characteristics, part two of empowerment questionnaire, and the third part of the hemodynamic variables record sheets before and after the Continuous Care model based rehabilitation program including measuring and recording of systolic and diastolic blood pressure, the pulse was measured by a physician and recorded by a researcher's assistant. Data analysis was done using SPSS software version 22 with independent t-test, t-test and Chi-square and at a significant level of less than 0.5%. Results: The findings showed that the two groups did not differ in terms of demographic and clinical variables and were completely homogeneous (P <0.05). Also, the findings showed that the training of cardiac rehabilitation program based on the Continuous Care model on patients' ability in the areas of control, perception, perceived susceptibility, and overall empowerment has a significant effect(P <0.05) and has no significant effect on perceived sensitivity(p>0.05). The results of t-test in cardiovascular indexes of patients showed that there was a significant difference between mean score of systolic pressure(P <0.05), diastolic pressure, cholesterol level and blood glucose level after intervention in both groups(P <0.05), but between mean pulse score after intervention in There are no significant differences between the two groups(p>0.05). Conclusion: The results indicated that the cardiac rehab program was based on a continuous care model on empowerment and cardiovascular indices in patients with ischemic heart disease. According to the results, it is recommended that nurses be trained on how to develop a cardiac rehabilitation program based on a Continuous Care model that can help empower patients, so that they can help their patients with these techniques.
