Abstract Background and aims: The rapid acceleration of industrialization and the lack of serious attention to industrial safety standards has led to an increase in the incidence of occupational accidents, while work-related accidents are one of the most important preventive health problems around the world. Investing in this important issue will well explain the economic justification of programs, policies, and investment in occupational health and safety. The purpose of this study was to investigate the most important causes of occupational accidents registered at the Department of Labor in the city of Qaen with the Pareto chart. Methods: The present study is a descriptive-analytic study that was based on data recorded by the Cooperatives, Labor, and Social Welfare Office of Qaen in a four-year period from 2013 to 2017 and analyzed with the aid of Excel software and Pareto charts. Results: According to the results of this study, a total of 30 accidents occurred, the most common type of incident was machine grip (43.33%) and fall and slip (26.67%), accounting for 70% of the incident be. In terms of level of literacy, subjects with cyclic degree (36.67%) had the highest incidence. (73.91%) of these accidents lead to injury and fracture and in 2% of cases it has resulted in death. Other findings from this study showed that (78.95%) of the accidents injured hands, feet and head. Conclusion: According to the results of this study, officials and employers should pay more attention to the causes of falling people, the fall of objects and the training of workers in working with a variety of industrial machinery and their risks, which contribute more to incidents with severe consequences, and, on the other hand, the need to use protective devices Individual training and instruction on how to use them effectively reduces injuries to the hands and feet, and to allocate more financial and human resources to prevent such accidents and their causes. Also, the use of various analyzes and diagrams such as Pareto as one of the methods for analyzing and identifying the causes of accidents can play an effective role in the management of work-related accidents, allocate the correct budget and time.
